Tarrant County Maternal & Infant Health Coalition’s Pilot Achieves 91% Closed-Loop Referral Rate for Local Mothers

In 2024, Best Place for Kids!®, on behalf of the Tarrant County Maternal & Infant Health Coalition, was awarded $250,000 by the donors at North Texas Community Foundation to execute a pilot to create a comprehensive, closed-loop system of care for mothers in Tarrant County. Based on an analysis of interviews with nearly 80 local mothers, as well as a resource map and gap analysis created by the members of the Tarrant County Maternal & Infant Health Coalition, the Coalition and Best Place for Kids!® team developed a unique high-tech, high-touch referral system leveraging the locally-built free Parent Pass® app as a universal access point for mothers, and Help Me Grow as phone line navigation for additional needs. After a year of the pilot, the Coalition was pleased to present the results of these efforts, which culminated in a 91% successful closed-loop referral rate for the 686 mothers who were served as a part of the pilot.
